Clinical Effectiveness Institute

Clinical effectiveness is not an optional enhancement to psychotherapy. It is a requisite for ethical practice.  The Clinical Effectiveness Institute provides training and consultation for behavioral health clinicians, supervisors, and organizations seeking to strengthen the quality of clinical care at the level where treatment actually succeeds or fails: the clinical encounter itself.

Behavioral health systems often track access, engagement, retention, documentation, client experience, discharge status, and outcomes. These measures matter. But beneath all of them is a harder-to-observe clinical question:  Can the clinician accurately assess what kind of therapeutic process this client can participate in right now — and calibrate the work accordingly?

When that assessment is missing, clinical work can become active but ineffective. A client may attend sessions, complete assignments, agree with the clinician, participate in treatment planning, or speak with insight, while little meaningful change occurs. The issue is not always motivation, resistance, diagnosis, or lack of intervention. Often, it is a mismatch between the clinical process being offered and the client’s current capacity to participate in it.  The Institute exists to help clinicians and organizations address that mismatch.

Training focus

The Clinical Effectiveness Institute offers training in clinical discernment, real-time calibration, case conceptualization, supervision, and behavioral health quality improvement.

Training topics include:

  • Assessing client regulation in real time.

  • Recognizing when clinical demand exceeds client capacity.

  • Calibrating stance, pacing, language, depth, structure, accountability, and intervention.

  • Strengthening clinical judgment beyond protocol selection.

  • Improving supervision and case consultation through capacity-based clinical thinking.

  • Connecting clinical effectiveness to ethical practice and quality improvement.

Current introductory training

Why Good Interventions Fail: Assessing Client Regulation in Real Time

This introductory half-day training is designed for substance-use and behavioral health clinicians and supervisors working with clients who may be ambivalent, dysregulated, trauma-impacted, relapse-prone, mandated, difficult to engage, or repeatedly cycling through treatment without meaningful change.

The training helps clinicians assess regulation as a functional clinical capacity: the client’s ability to remain organized enough to participate in the therapeutic encounter, tolerate emotional demand, stay in contact, reflect when possible, access agency, and carry something forward beyond the session.

Participants learn to recognize regulation mismatch and recalibrate the clinical process before treatment becomes performative, ineffective, or destabilizing.

For organizations

The Institute is especially relevant to behavioral health organizations interested in strengthening clinical quality, supervision, provider effectiveness, treatment-plan utility, engagement, retention, and meaningful client participation in care.

Training can be offered to clinical teams, supervisors, substance-use programs, community mental health providers, and county-contracted behavioral health organizations.

Sarah Ozol Shore, MS, is a clinical quality improvement professional, behavioral health clinician, and clinical educator. Her background includes quality and performance improvement work in community mental health, substance-use treatment, behavioral health systems, and hospital-based care, along with direct clinical experience as a therapist.

Her work centers on clinical effectiveness as a requisite for ethical practice. She is developing the Clinical Effectiveness Institute as a home for advanced training in clinical discernment, supervision, and behavioral health quality improvement.
